Physical Readiness Questionaire (PAR-Q)

Regular physical activity is safe for most people. However, some individuals should check with their doctor before they start becoming much more physically active. Please read the 7 questions below carefully and answer each one honestly: YES or NO.

Please Note: If you answer "YES" to one or more questions, you must consult with your doctor BEFORE starting the Coaching program to gain medical clearance. Tell your doctor about the PAR-Q and which questions you answered YES to.
